Terms & Limitations
This program is a discount plan, not a dental insurance plan. The annual premiums must be paid with either cash, check, or credit card. This plan is only valid at Magnolia Dental Arts. It cannot be used at another dental office:
• In conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
• For treatment that, in the sole opinion of our doctors, lies outside the realm of their capabilities.
• For referrals to a specialist
• For hospitalization or hospital charges of any kind
• For costs of dental care that are covered by automobile medical insurance
• For services related to injuries covered under workers’ compensation
Program Guidelines
• Patient’s portion of the bill is due on the day of service.
• The plan cannot be used in conjunction with another dental plan.
• CareCredit may not be used to pay for the Club premium…
• No refunds of premiums will be issued if a participant chooses not to use the Club plan.
